Difficult Airway Management in Adult Coronavirus Disease 2019 Patients: Statement by the Society of Airway Management
Anesthesia and AnalgesiaLorraine Foley, et al.
2021-10-01
The coronavirus disease 2019 (COVID-19) disease, caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), often results in severe hypoxemia requiring airway management. Because SARS-CoV-2 virus is spread via respiratory droplets, bag-mask ventilation, intubation, and extubation may place health care workers (HCW) at risk. While existing recommendations address airway management in patients with COVID-19, no guidance exists specifically for difficult airway management.
COVID-19 Putting Patients at Risk of Unplanned Extubation and Airway Providers at Increased Risk of Contamination

Coronavirus 2019 puts patients at increased risk of experiencing unplanned extubation (UE), and both UE and subsequent reintubation put providers at increased risk of viral contamination. The increased patient surge that has occurred during this COVID-19 pandemic and the associated less-than-ideal provider/patient ratios put patients at increased risk for UE. In addition, patient proning, a common procedure in critically ill COVID-19 adult respiratory distress syndrome patients, is a risk factor for UE.
Alternate airway strategies for the patient with morbid obesity

Anesthesiologists are encountering patients with morbid obesity in increasing numbers and as many as one in 3 of the regular caseload may be obese. The specialty of bariatric anesthesiology is emerging as a valuable resource in addressing the physiological problems and metabolic complications of the obese patient. Many institutions have appointed a clinical lead anesthesiologist in this specialty, especially with expertise in airway management of the obese patient.
